President's Message: How Engaged Are You?

It was so nice to see so many of you at the ASHP Midyear meeting. When I looked around, it reinforced how proud I am to be a member of MSHP. Many of our members were involved in numerous activities and even presenters at the meeting. There was MSHP representation in the Pharmacy Student Programming, New Practitioner Programming, and Pharmacist CE Sessions. I also saw many of you at the State Affiliate Leader’s Conference, Poster Sessions, Residency Showcase and the Missouri Reception that we hosted in conjunction with St. Louis College of Pharmacy and UMKC School of Pharmacy. We are fortunate to have so many engaged individuals in the pharmacy profession that are members of our organization. The ASHP meeting is a great time to display the diverse talents we all possess on a national level.
In addition to this talent on display at the national level, we are fortunate to have a plethora of members willing to display their talents at our annual spring meeting. I’ve had a sneak peak at the programming for this meeting and I know you will be excited. One program I would like to highlight will deal with the Pharmacy Practice Model Initiative (PPMI). Many of you have seen an e-mail requesting each of your practice sites to complete the online PPMI self-assessment. I encourage you to complete this soon. The results from hospitals that will have submitted their information in both Kansas and Missouri will be utilized in a PPMI session. At my facility, we are completing the survey, which in and of itself is a good tool to get you and your coworkers thinking about “moving the needle” of our profession forward. After you complete this assessment, I’d love to hear more about what changes have been made at your place of employment.
I’ve mentioned how many individuals within our organization are engaged in the profession. I’ve also suggested what you can do now within your place of employment to help move the pharmacy profession forward. I encourage you to consider becoming involved on a state level with MSHP if you are not already. There are many opportunities for interaction with others and impacting the way that the profession of pharmacy should move. Stroke Prevention in Atrial Fibrillation: Rivaroxaban, Dabigatran, and Warfarin

Warfarin, a vitamin K antagonist, has previously been the drug of choice for reducing the risk of stroke and systemic embolism in patients with non-valvular AF. When using warfarin, a patient’s INR should be monitored closely and dose adjustments may be required to keep the patient within the therapeutic range. Many different medications and foods can interact with warfarin, causing INR levels to fluctuate, and necessitating medication and dietary modifications. Due to the complexity of conventional warfarin therapy, new anti-coagulants with different mechanisms of action have been developed. The new oral anti-coagulant agents currently approved by the FDA for this indication are rivaroxaban (Xarelto) and dabigatran (Pradaxa). Read More
Critical Evaluation of: "CYP2C19 Genotype, Clopidogrel Metabolism, Platelet Function, and Cardiovascular Events: A Systematic Review and Meta-Analysis"

Following the recently published meta-analysis that questions the association between CYP2C19 genotypes and cardiovascular outcomes in patients taking clopidogrel it is important to revisit the evidence for genotyping these patients. Although studies have both supported and refuted an association between CYP2C19 genotypes and outcomes in clopidogrel patients, a meta-analysis merging this data to draw a single conclusion is likely to have important limitations. In response to conflicting evidence new hypotheses about the interaction between the CYP2C19 genotypes and clopidogrel are also emerging, such as the hypothesis put forth by M. J. Cohen suggesting that clopidogrel may have thrombogenic metabolites responsible for poor outcomes that are more prevalent when clopidogrel is taken with proton pump inhibitors. We aim to summarize the evidence of pharmacogenomics interactions in clopidogrel patients, discuss the impact on healthcare dollars, evaluate the Holmes, et al meta-analysis, gain insight through experts in the field, and discuss additional relevant pharmacogenomic factors. The Role of Statin Therapy to Acutely Prevent Myocardial Damage in Patients Undergoing Percutaneous Coronary Intervention

Myocardial damage, or necrosis, can be assessed by elevated levels of creatine kinase-MB (CK-MB), troponin-I, and myoglobin in the blood when cellular injury has occurred. Elevation of these cardiac markers from damage during percutaneous coronary intervention (PCI) occurs in up to 31% of cases. These peri-procedural elevations may occur for several reasons, including distal embolization of plaque or thrombus compromising coronary collaterals, increased sympathetic vasoconstrictor release, and side-branch occlusion, all of which can cause small zones of necrosis in the myocardium due to lack of coronary blood flow, referred to as a myocardial infarction (MI).2 Elevated cardiac markers after PCI are associated with an increased risk of cardiac death, mortality, bypass surgery, and repeat coronary interventions. Awards Time
It's time once again for you to nominate your peers for a variety of awards. Below are descriptions of the awards. If you are interested in nominating someone for an award, please email Sara Neiswanger a nomination letter and the CV of the person for whom you are nominating.
MSHP Pharmacist of the Year Award
The MSHP Pharmacist of the Year Award is given annually, if a recipient is selected, to a pharmacist at the Annual Meeting of MSHP. The recipient should meet a combination of the criteria listed below, or demonstrates exceptional performance in one area. The award may be given for sustained performance or for outstanding performance in the year the award is given. The award recipient must be a MSHP member and demonstrate a high level of integrity, ethical conduct, and professionalism. Criteria considered are accomplishments or outstanding performance in any of the following areas:
1) Health-system pharmacy practice, administration, or education.
2) Clinical research in health-system pharmacy practice or patient care.
3) Service or contributions to health-system pharmacy practice.
4) Service or contributions to the community.
5) Service or contributions to public health.
6) Activities that improve the safety or effectiveness of medication use in health systems.
Nominees will be accepted from the MSHP membership and Board of Directors. Nominations should include written information describing how the accomplishments and activities of the nominee match the award criteria.
MSHP Outstanding Volunteer of the Year Award
Each year, the Awards Committee seeks recommendations from the MSHP Board and membership to recognize outstanding volunteers. The purpose is to give recognition and thanks for service to MSHP. Qualifications will include:
1) MSHP membership in good standing, and
2) Significant contribution to a committee, or
3) Significant contribution to completion of strategic plan tactics, or
4) Significant contribution of time in completion of duties.
The current President, President-Elect, and Past President should not be considered for this recognition.
MSHP Rising Star Award
Each year, the Awards Committee seeks recommendations from the MSHP Board and membership to recognize recent graduates who have become involved in MSHP. Qualifications will include:
1) MSHP member in good standing; and
2) Less than 5 years after graduation or pharmacy student; and
3) Served as an officer, committee chair, committee member in MSHP, GKCSHP, MMSHP or STLSHP; or
4) Presented a poster at a pharmacy state or national meeting; or
5) Authored an article in the MSHP newsletter
Individuals may be nominated each year they qualify. Individuals are recognized at the Annual Meeting of the organization.
KCHP/MSHP Annual Meeting 2012 Poster Deadline Approaching
The deadline for Poster Abstract submission for those wanting to enter a poster in the KCHP/MSHP 2012 Annual Meeting has been set for February 15th. Acceptance notifications will go out in early March. To submit an abstract for a poster, please fill out our online submission form.
Exhibit Registration Open for KCHP/MSHP Annual Meeting 2012
Exhibit booth registration is now open for the KCHP/MSHP Annual Meeting 2012 in Kansas City in April. Any company wishing to exhibit needs to register in advance. KCHP members are encouraged to pass along the information and online registration link to vendors in their facilities. Exhibit registration is available here.
MSHP to Attend ASHP Affiliated State Society Executive Meeting
ASHP hosts its annual Affiliated State Society Executive Meeting February 12-13th at ASHP’s headquarters in Bethesda, MD, and KCHP will be there. The purpose of this meeting is to:
- facilitate communication between ASHP state affiliate executives and ASHP’s elected leadership and staff
- build awareness and deeper understanding of key practice and policy issues affecting hospital and health-system pharmacy practice
- facilitate enhanced utilization of ASHP resources and participation in ASHP initiatives among ASHP affiliates
- enhance ASHP's awareness of and responsiveness to challenges and issues affecting state affiliate organizations and the health-system pharmacy practitioners they serve
- facilitate communication and information sharing among ASHP's affiliate executives
MSHP Account Executive Sara Neiswanger will be attending the meeting on behalf of MSHP. She and other state executives in attendance will receive updates from many ASHP divisions, including Affiliate Relations and Government Affairs. In addition, Board Certification Trends and the Pharmaceutical Supply Chain will be discussed. Stay tuned for a recap of the event in March!

Student of the Year: Rachel Sass, UMKC School of Pharmacy, Student Extern at Providence Medical Center
Rachel was elected student of the year because of her drive for learning and great “can do” attitude towards her work and personal life. During her clinical rotation at Providence Medical Center, she independently completed several projects for the department and excelled at clinical decision making that kept the best interest of the patient in mind. Rachel is a team player and formed great relationships with the pharmacy department staff and other pharmacy students while at Providence Medical Center.
Technician of the Year: Patty McCombs, Certified Pharmacy Technician II, Saint Luke’s Hospital of Kansas City
Patty was elected as the technician of the year because of the professionalism she displays and her always positive attitude. She is self-motivated and can be counted on to successfully work in any role she is assigned. She has moved in to her new role as the Narcotic technician seamlessly as she is a very quick learner. Patty is a team player, and it is evident that she strives to keep patients a priority.

MMSHP Call for Nominations – President-Elect and Secretary/Membership Chair
Deadline to Submit a Nomination: January 31, 2012
PRESIDENT-ELECT (then President, then Immediate-Past President) – 3 year term
The President-Elect shall perform the duties of the President, whenever the President shall be unable to do so. The President-elect is a member of the Executive Committee and shall serve as its vice-chairperson. The President-elect shall be an ex-officio member of all committees and will serve on the Program Committee. During his/her term of office, he/she shall become familiar with all the activities of the Society, in preparation for assuming the office of president the following year.
SECRETARY/MEMBERSHIP CHAIR – 2 year term
The Secretary/Membership Chair is responsible for recording all activities of the Society, shall conduct the correspondence of the Society, and shall present all correspondence received for the Society, at the next regular meeting of the Executive Committee. The Secretary/Membership Chair shall serve for a period of two (2) years. The Secretary/Membership Chair shall take minutes of all meetings and forward it to the Executive Committee. The Secretary/Membership Chair shall be responsible for encouragement of the Society membership. The Secretary/Membership Chair shall also be responsible for notifying members of delinquent membership status and maintaining a list or roll of active members.
Additional Information
- The officers shall serve a term beginning in May of the year they are elected for the duration of their term ending in April.
- Nominee must be a current MMSHP member. Only active, pharmacist members may run for officer position.

The function of the President-Elect is to participate in StLSHP activities and establish objectives in preparation for the presidential year.
Duties
- Perform the duties of the office of the President whenever the President is unable to do so.

- Help plan and develop monthly programs for the organization.
- Be available to assist the President in travel appearances when necessary.
- Be an ex-officio member of all committees.
StLSHP Secretary
The function of the Secretary is the custodian of the Society's important documents, and is responsible for recording all activities of StLSHP.
Duties
- Conduct the correspondence of the Society, and present all correspondence received.
- Be a member of the Executive Board.
- Take minutes at all official StLSHP activities for the Executive Board.

DEA Releases Statement Regarding Carisoprodol Prescriptions
The Drug Enforcement Administration (DEA) made carisoprodol (Soma®) a Schedule IV drug effective January 11, 2012. As of January 11, 2012, carisoprodol may only be dispensed in accordance with the Controlled Substances Act. The DEA has officially issued a Guidance Statement regarding carisoprodol prescriptions that is now available on its Website.
The Missouri Board of Pharmacy has received multiple inquiries regarding legally valid prescriptions for carisoprodol that were written before January 11, 2012, that have valid refills remaining. According to the Bureau of Narcotics and Dangerous Drugs and the DEA's Guidance Statement, licensees may continue to dispense these prescriptions if the prescription was written by an authorized DEA registration with a DEA number. If the prescriber does not have a DEA number, those prescriptions should be canceled and not dispensed. Licensees are encouraged to communicate this information to pharmacy staff to ensure compliance.

ASHP Disappointed With Statement On Pharmacists By Family Physicians
The American Academy of Family Physicians (AAFP) recently released a position paper that seeks to define the role of pharmacists in working with family physicians. The paper, entitled “Pharmacists,” expresses a number of views that do not reflect contemporary pharmacy practice or the central tenets of interdisciplinary patient care that are supported by a number of physician organizations and other prominent health care groups. Although the position paper acknowledges the value of the patient care pharmacists provide through collaborative drug therapy management (CDTM), it goes on to state that in the context of prescribing “the pharmacy professional is in the position to dispense the prescription written by the physician.” This statement fails to recognize that under CDTM agreements and hospital privileging that pharmacists routinely initiate and modify drug therapy as integrated members of the patient care team. ASHP plans to work with colleagues in other pharmacy professional associations to respond to this statement and will request a meeting with AAFP leadership to discuss the position paper. Read More
PPMI Survey Deadline Approaching
The American Society of Health-System Pharmacists (ASHP) is spearheading the Pharmacy Practice Model Initiative (PPMI) across the country. This Initiative is designed to reflect the changes the profession has seen over the past decades and to better prepare it for changes on the horizon. Learn more about PPMI.
As the recognized ASHP affiliate, the Missouri Society of Health-System Pharmacists (KCHP) wants to lead Missouri hospital pharmacies to complete this survey soon and place the state on the leading edge of pharmacy practice. This tool needs only be completed by one representative within your department, but may require input from multiple colleagues. It should take no more than an hour, and may be completed in more than one sitting. You may also print the PDF survey to complete it on paper prior to entering the compiled data. You will receive a summary of your survey from ASHP once completed.

New Drug Treats Toxic Methotrexate Concentrations
FDA on Tuesday approved the marketing of glucarpidase, or Voraxaze, for the treatment of toxic plasma methotrexate concentrations in patients whose kidneys are not eliminating the drug at a normal rate. The drug's labeling (PDF) notes that the recombinant bacterial enzyme is not for use in patients whose renal function is normal or only mildly impaired. Read More

As pharmacy practice evolves, so do the strategies that help drive those changes—including the ambitious and comprehensive Pharmacy Practice Model Initiative (PPMI), which has supplanted ASHP's 2015 Initiative. Launched in 2003, ASHP's Health-System Pharmacy 2015 Initiative was a member-driven plan "structured to help us reach the ASHP vision for pharmacy practice in hospitals and health systems," said ASHP's Douglas Scheckelhoff, vice president of professional development.
